Care Manager

This is a 13-week contract position.

Schedule

Day Shift: 5x8-Hour (08:00 - 16:30). A variable 5-day workweek with department operations occurring 7 days per week. Days off will vary and are not guaranteed to be back-to-back.

Job Description

The Care Manager will be responsible for care coordination and discharge planning for patients. This role requires assessing patient needs, developing treatment plans, and participating in family meetings. The Care Manager must be competent in arranging necessary services such as DME and transport services independently.

Responsibilities

• Perform care coordination and discharge planning for patients.
• Assess patient needs and organize family meetings.
• Develop treatment plans in collaboration with the healthcare team.
• Coordinate with various services including DME, SNF, and ambulance transport.
• Handle documentation and charting using the Epic system.

Required Experience / Certifications / Licensure

• 2 years of acute medical Care Manager experience.
• California state license required; pending licenses accepted if the candidate is fully licensed upon start date.
• Experience with assessing patients and care coordination.
• Epic charting system experience is mandatory.
