Day shift, 5x8 hours (08:00 - 16:30). Weekend coverage typically required every other weekend.

Job Description

The Care Manager will be responsible for care coordination and discharge planning in an acute care setting. This position requires effective management of patient cases, ensuring that care is aligned with evidence-based guidelines and meeting CMS standards.

Responsibilities

• Perform care coordination and discharge planning for a caseload of up to 25 patients.
• Assess patients and determine medical necessity according to evidence-based guidelines.
• Collaborate with interdisciplinary teams to develop and implement plans of care.
• Manage patient documentation and charting using the Epic system.
• Adhere to CMS regulations and HIPAA guidelines.

Required Experience / Certifications / Licensure

• Two years of acute medical Care Manager experience with a focus on care coordination and discharge planning.
• Current California state license; pending licenses accepted if obtained by start date.
• Experience with computerized documentation, specifically the Epic charting system.
