Acute Care Case Manager

This is a 13-week contract position.

Schedule

Day shift, 5x8-hour schedule from 08:00 - 16:30. Variable days off with efforts made for consecutive days off.

Job Description

The Acute Care Case Manager will focus on care coordination for patients in an acute care setting. This role involves assessing patients, participating in family meetings, and developing treatment plans, ensuring effective discharge planning is in place.

Responsibilities

• Perform case management and care coordination for patients, focusing on acute medical needs.
• Conduct assessments and participate in family meetings to address patient care plans.
• Organize and coordinate patient discharges and follow-up services.
• Ensure compliance with CMS regulations and advance directive protocols.
• Arrange durable medical equipment, skilled nursing facilities, and necessary transport services.

Required Experience / Certifications / Licensure

• Minimum of 2 years of acute care case management experience.
• Current CA state license is required (pending license accepted, but must be licensed upon start date).
• Experience with Epic electronic health records system is mandatory.
• Strong understanding of care management for diverse patient populations, including those with complex needs.
