This is a 13-week contract position.

5x8.0 Shift starting October 19th, 60-minute evaluations followed by 30-minute follow-ups.

Job Description

The Outpatient Physical Therapist evaluates assigned patients and develops, implements, and modifies treatment plans according to the patients' needs, goals, and physician direction. This role includes delegating and directing all discipline-specific therapy services for assigned patients while ensuring the design of functional treatment programs that facilitate an effective return to community activities. The Physical Therapist also serves as an educator for patients, families, students, coworkers, and themselves while performing other professional duties associated with patient care and therapy service development. Adherence to the organization's Mission Statement and Core Values is essential, as is maintaining compliance with all relevant guidelines and safety protocols.

Responsibilities

  • Complete comprehensive evaluations of assigned patients and develop appropriate treatment plans and goals focused on progressing patients towards discharge within an established length of stay.
  • Document medical records clearly, concisely, accurately, and timely in compliance with state, federal, and clinic guidelines.
  • Ensure care is provided under valid physician orders and within insurance approval.
  • Participate in the student program by providing supervision and completing evaluations.
  • Create an individualized professional development plan and pursue professional growth through education and involvement in professional activities.
  • Adhere to discipline-specific State Practice Act and maintain professional licensure in good standing.
  • Understand and engage in performance improvement initiatives within the organization.

Required Experience / Certifications / Licensure

  • Degree in Physical Therapy from an accredited school.
  • Six months to one year of outpatient clinical experience.
  • Current CPR certification.
  • Active state licensure as a Physical Therapist.
