Outpatient Physical Therapist - 5x8 Schedule

Job Description

We are seeking a dedicated Outpatient Physical Therapist to evaluate and treat patients in accordance with their needs, goals, and physician direction. This role involves developing, implementing, and modifying treatment plans tailored to enhance patient recovery and facilitate a return to community activities. As a Physical Therapist, you will serve as an educator to patients, families, students, and colleagues while adhering to the highest standards of professionalism and compliance with organizational policies.

Responsibilities

• Conduct comprehensive evaluations of assigned patients.
• Develop and implement effective treatment plans focused on patient progress.
• Maintain clear and accurate medical record documentation, inclusive of goal writing and functional assessments.
• Provide patient care under valid physician orders and within insurance approvals.
• Participate in student training programs as assigned.
• Engage in ongoing professional development and complete state-mandated continuing education requirements.
• Contribute to organizational performance improvement initiatives.

Required Experience and Certifications

• Degree in Physical Therapy from an accredited school.
• 6 months to 1+ year of outpatient clinical experience preferred.
• Current CPR certification is required.
• State licensure in good standing as a Physical Therapist.

Schedule

This position operates on a 5x8 schedule, starting as soon as possible. Each patient evaluation is 60 minutes, followed by 30-minute follow-up sessions.
