Outpatient Physical Therapist

Job Description

We are seeking a dedicated Outpatient Physical Therapist to evaluate and develop treatment plans in alignment with patient needs and goals. This role involves comprehensive evaluations, implementation of treatment strategies, and ongoing modifications based on patient progress. The Physical Therapist will also educate patients, their families, and colleagues while upholding the highest standards of care and compliance in line with organizational values.

Responsibilities

• Conduct comprehensive evaluations of assigned patients and develop individualized treatment plans focused on achieving set goals.
• Complete and maintain accurate medical records, including documentation, goal writing, and charge entry in compliance with relevant guidelines.
• Ensure care is administered under valid physician orders and within approval parameters.
• Participate in the education of students and coworkers by providing guidance and evaluations.
• Commit to professional development through continuing education and participation in professional activities.
• Maintain licensure in good standing and adhere to the prescribed ethical conduct and standards.
• Engage in performance improvement activities to enhance service quality.

Required Experience / Certifications / Licensure

• Degree in Physical Therapy from an accredited institution.
• CPR certification.
• 6 months to 1+ year of outpatient clinical experience preferred.
• Current state licensure to practice as a Physical Therapist.

Schedule

This is a full-time outpatient role with a schedule of 5 days a week, 8-hour shifts. A commitment to a 6-month contract is desired.
