Job Description

We are seeking a dedicated and experienced Registered Nurse for our Case Management unit. As a float unit, you will collaborate with a multidisciplinary team to ensure comprehensive care and effective discharge planning for patients.

Responsibilities

• Develop and implement individualized care plans for patients.
• Coordinate patient care with various healthcare providers.
• Facilitate complex discharge planning and resource management.
• Utilize EMR systems (Meditech, Midas) for documentation and patient data management.
• Participate in team meetings to discuss patient progress and outcomes.

Required Experience / Certifications / Licensure

• Current RN license in the state of Florida.
• Experience with Meditech and Midas systems is mandatory.
• Strong background in case management practices.
