Shift: 08:30 - 17:00 with alternating weekends.

Job Description

We are seeking a dedicated Registered Nurse specialized in Case Management to join our team. The ideal candidate will manage patient care effectively and ensure the smooth operation of patient services within the unit.

Responsibilities

• Provide case management services to patients to ensure quality care.
• Collaborate with interdisciplinary teams to develop and implement care plans.
• Monitor patient progress and adjust treatment plans as necessary.
• Educate patients and families on health management and available resources.

Required Experience / Certifications / Licensure

• Active RN license in the state of Florida.
• Experience in acute care Case Management is required.
• Familiarity with EMR systems, specifically Meditech and Midas, is preferred.
