Job Description

The Outpatient Physical Therapist evaluates assigned patients and develops, implements, and modifies treatment plans in accordance with patients' needs, goals, and physician direction. This role includes the delegation and direction of all discipline-specific therapy services for assigned patients, ensuring the design of functional treatment programs that facilitate effective returns to community activities. Additionally, the Physical Therapist serves as an educator to patients, families, students, coworkers, and themselves.

Responsibilities

• Conduct comprehensive evaluations of assigned patients.
• Utilize clinical reasoning to develop appropriate treatment plans and goals, aiming to progress patients towards discharge.
• Document medical records in a clear, concise, accurate, and timely manner.
• Ensure all care is provided under valid physician orders and within insurance approval guidelines.
• Participate in student programs, providing supervision and positive learning environments.
• Create and actively pursue an individualized professional development plan.
• Adhere to discipline-specific State Practice Acts and maintain professional licensure in good standing.
• Understand and follow performance improvement initiatives in accordance with the organization’s methodologies.

Required Experience / Certifications / Licensure

• Degree in Physical Therapy from an accredited school.
• 1+ years of outpatient clinical experience.
• Current CPR certification.
• Active and applicable state licensure for practicing physical therapy.
• Previous experience with the agency or Select Medical is highly desired.
