Home Health and Hospice Physical Therapist

This is a 13-week contract position.

Schedule

Monday to Friday, 8 AM to 5 PM, with holiday coverage as needed.

Job Description

We are seeking a dedicated Physical Therapist with experience in OASIS, home health, and hospice care. The ideal candidate must be adaptable to changes in schedule and have experience traveling for patient home visits.

Responsibilities

• Conduct patient assessments and evaluations in a home setting.
• Develop and implement individualized treatment plans.
• Manage and adjust to changes in patient schedules and care needs.
• Document patient progress in the EMR system (HealthWyse).
• Collaborate with healthcare professionals to coordinate patient care.

Required Experience / Certifications / Licensure

• Active Physical Therapist license.
• Demonstrated experience in OASIS documentation.
• Prior experience in home health and hospice settings.
• Ability to manage travel for home visitations efficiently.
