5x8.0 schedule with flexible hours including 2 evenings until 7 PM.

Job Description

The Outpatient Physical Therapist evaluates all assigned patients and develops, implements, and modifies treatment plans aligned with the patients' needs, goals, and physician directions. This role involves the delegation and direction of all discipline-specific therapy services for assigned patients while ensuring the design of functional treatment programs that facilitate effective returns to community activities. The therapist serves as an educator for patients, families, students, coworkers, and themselves, and performs professional duties related to patient care and the development of therapy services.

Responsibilities

• Complete comprehensive evaluations of assigned patients and utilize clinical reasoning to develop appropriate treatment plans and goals.
• Document medical records clearly, concisely, accurately, and in a timely manner while adhering to guidelines.
• Ensure care is provided under signed, valid physician orders and within insurance approval.
• Participate in the student program, providing a positive learning environment and supervision.
• Actively pursue professional development and apply learned information to meet strategic objectives.
• Maintain compliance with state practice acts and professional codes of conduct.
• Demonstrate awareness of and contribute to the organization’s performance improvement initiatives.

Required Experience / Certifications / Licensure

• Degree in Physical Therapy from an accredited school.
• Minimum of 1+ year of outpatient clinical experience.
• Current CPR certification.
