Job Description

We are seeking a compassionate and dedicated Registered Nurse (RN) with over 2 years of recent and relevant home care experience. The RN will be responsible for conducting 25-30 visits per week, including a mix of start of care and revisits. Strong proficiency in OASIS and Epic systems is required.

Responsibilities

• Conduct patient assessments and implement care plans.
• Perform OASIS documentation and ensure compliance with home health regulations.
• Administer medications and treatments as prescribed.
• Collaborate with patients, families, and interdisciplinary teams to optimize patient outcomes.

Required Experience / Certifications / Licensure

• Active Registered Nurse (RN) license.
• Minimum of 2 years of recent home care experience.
• Experience with OASIS and Epic systems.
