Job Description

We are seeking a compassionate and dedicated Licensed Practical Nurse (LPN) to join our rehabilitation team. Our facility specializes in the recovery of individuals who have experienced severe injuries or illnesses, providing tailored rehabilitation programs to enhance patient health and quality of life.

Responsibilities

• Provide direct patient care and monitor patient progress during rehabilitation.
• Assist in administering medications and treatments as prescribed by healthcare providers.
• Collaborate with multidisciplinary teams to develop and implement personalized care plans.
• Maintain accurate and timely documentation using electronic health records (EHR) systems.
• Educate patients and families about rehabilitation processes and expected outcomes.

Required Experience / Certifications / Licensure

• Current LPN licensure and BLS certification.
• A minimum of 2 years of experience in a healthcare setting, preferably in rehabilitation or medical-surgical environments.
