Job Description

The Licensed Practical Nurse (LPN) will provide direct and indirect skilled nursing care for assigned residents, ensuring the delivery of age-appropriate care and maintaining clear communication with the healthcare team. The LPN is responsible for adhering to policies and procedures under the supervision of a Registered Nurse (RN). This role requires a good understanding of patient growth and development to provide care tailored to the needs of adult and geriatric patient populations.

Responsibilities

• Collect ongoing data relevant to residents' physical and psychosocial conditions, and communicate findings to RNs and the interdisciplinary team.
• Contribute to the creation of developmentally appropriate and culturally relevant care plans, involving residents and families in the process.
• Evaluate resident outcomes and suggest necessary revisions to care plans, indicating the need for referrals as appropriate.
• Participate in the mentoring and orientation of new staff, maintaining professional standards of nursing practice.
• Ensure safety and efficacy in nursing care delivery, actively contributing to a healthy and safe environment for residents.

Required Experience / Certifications / Licensure

• Valid New York State LPN Licensure.
• BLS certification (AHA or RQI specifically).
• Minimum of 1 year of experience serving in an LPN capacity at a Long-Term Care (LTC) facility.
