Job Description

The Licensed Practical Nurse (LPN) will provide direct and indirect skilled nursing care for residents, ensuring that care is appropriate to the age-specific needs of the adult and geriatric patient populations. The LPN will work under the supervision of a Registered Nurse (RN) and maintain collaborative relationships with other nursing staff, physicians, and ancillary staff.

Responsibilities

• Collect data relevant to residents' physical and psychosocial conditions and communicate findings to the RN and Interdisciplinary Team.
• Contribute to creating developmentally appropriate and culturally relevant care plans, reflecting nursing diagnoses and interventions.
• Evaluate resident outcomes and recommend revisions to care plans as necessary.
• Collaborate with residents/families and healthcare teams to ensure comprehensive care and customer satisfaction.
• Maintain current knowledge and competency in nursing practice, participating in mentoring and orientation of staff.
• Provide leadership in the professional practice setting under the direction of the RN.
• Ensure safety and quality in care while considering cost-effectiveness.

Required Experience / Certifications / Licensure

• Current NY LPN Licensure
• Basic Life Support (BLS) certification
• Minimum of 1 year of experience serving in an LPN capacity, preferably in a Long-Term Care (LTC) facility
