LPN Full-Time Position

Schedule

3:00 PM - 11:30 PM, 5 days a week, every other weekend and holiday required.

Job Description

The Licensed Practical Nurse (LPN) will demonstrate the necessary knowledge and skills to provide care appropriate to the age of patients in the assigned unit. The individual must be able to assess data reflective of the patient's status and identify each patient's requirements relative to their age-specific needs. The LPN will provide direct and indirect skilled nursing care for assigned residents while supervised by a Registered Nurse.

Responsibilities

• Collect ongoing data relevant to residents' physical and psychosocial condition and communicate findings to RN and interdisciplinary team.
• Contribute to the creation of developmentally appropriate and culturally relevant care plans.
• Collaborate with residents/families and team members to ensure comprehensive care.
• Maintain current knowledge and competency in nursing practice and participate in the mentoring and orientation of staff.
• Provide leadership in a professional practice setting under the direction of the RN.

Required Experience / Certifications / Licensure

• New York LPN Licensure and BLS certification (AHA or RQI only).
• Minimum of 1-year experience serving in an LPN capacity, preferably at a long-term care facility.
