Licensed Practical Nurse (LPN) - Full-Time Nights

Job Description

The LPN must demonstrate the knowledge and skills necessary to provide care appropriate to the age of the patients served on his or her assigned unit. The individual must possess the ability to assess data reflective of the patient's status and interpret the appropriate information needed to identify each patient's requirements relative to his or her age-specific needs, and to provide the care needed by adult and geriatric patient groups.

Responsibilities

• Collect data relevant to resident's physical and psychosocial condition on an ongoing basis and communicates findings to the RN and other members of the Interdisciplinary Team.
• Contribute to the creation of a developmentally age-appropriate and culturally relevant plan of resident care reflecting nursing diagnosis and interventions.
• Recommend revisions to the plan of care after evaluation of resident outcomes and indicate the need for referrals to ensure all aspects of the resident's care have been addressed.
• Maintain current knowledge and competency in Nursing Practice and participate in the mentoring/orientation of staff.
• Provide leadership in the professional practice setting under the direction of the RN.

Required Experience / Certifications / Licensure

• New York LPN Licensure.
• BLS Certification (AHA or RQI only).
• Minimum of 1 year experience as an LPN in a Long-Term Care (LTC) Facility.

Schedule

This position requires full-time night shifts, every other weekend, and holiday availability.
