Day shift, 5x8 hours (07:30 AM - 04:00 PM), with weekend coverage required every other weekend.

Job Description

The Case Manager RN will be responsible for managing patient care through effective discharge planning and coordination. The ideal candidate will have acute care experience and be proficient in using Epic for documentation.

Responsibilities

• Assess patient needs and develop appropriate plans of care.
• Coordinate with interdisciplinary teams to ensure seamless patient transitions.
• Utilize Epic for documentation and compliance with regulations.
• Conduct ongoing evaluations to determine the effectiveness of care plans.

Required Experience / Certifications / Licensure

• Valid Alaska RN License required (in hand for auto offer).
• 1 year of nursing experience, with emphasis on acute care.
• Basic Life Support (BLS) certification required.
• ACM certification preferred.
• Experience with Epic electronic medical records is required.
