This is a 13-week contract position.

Schedule

Shift: Day 5x8-Hour (07:30 - 16:00) with weekend requirements (every other weekend or 4 weekend days in a 4-week schedule).

Job Description

The Case Manager RN will participate in discharge planning, care coordination, and utilization review in an acute care hospital setting. Responsibilities will include assessing patient needs, collaborating with interdisciplinary teams, and adhering to healthcare regulations and guidelines.

Responsibilities

• Manage and coordinate patient care from admission to discharge.
• Conduct assessments and create individualized care plans.
• Ensure compliance with CMS and Joint Commission standards.
• Collaborate with various departments and healthcare professionals to optimize patient outcomes.
• Utilize Epic for charting and documentation.
• Participate in case management meetings and multidisciplinary rounds.

Required Experience / Certifications / Licensure

• Active AK RN license required.
• Must have BLS certification; ACM certification preferred.
• Experience in acute care settings is required; experience using Epic is a must.
• Previous case management experience is preferred.
