This is a 13-week contract position.

Schedule

This position requires a Day shift, working five 8-hour days, with varying start times between 07:00 and 08:30. Weekend work is required every other weekend or four weekend days within a four-week schedule.

Job Description

The Case Manager RN will be responsible for coordinating patient care, ensuring proper discharge planning, and case management within an acute care hospital setting. The role will require effective communication and collaboration with interdisciplinary teams to facilitate quality patient outcomes.

Responsibilities

• Manage and coordinate patient care and discharge planning.
• Conduct assessments to determine medical necessity per evidence-based guidelines.
• Provide care coordination in compliance with CMS regulations.
• Utilize Epic for charting and documentation of patient information.
• Collaborate with various healthcare professionals and teams.

Required Experience / Certifications / Licensure

• Active Alaska RN License is required.
• BLS certification is preferred.
• Experience in an acute care hospital setting is essential.
• Epic experience required, with case management experience preferred.
• ACM Certification is preferred.
• Minimum of 1 year experience in case management or related field.
