Physical Therapist - Outpatient

Job Description

The Physical Therapist evaluates assigned patients and develops, implements, and modifies treatment plans based on individual patient needs, goals, and physician direction. This role includes the delegation and direction of all discipline-specific therapy services for those patients, ensuring effective community activity reintegration. The Physical Therapist also serves as an educator for patients, families, students, and coworkers while adhering to professional standards and compliance expectations.

Responsibilities

• Conduct comprehensive evaluations of assigned patients and develop individualized treatment plans.
• Document patient care and treatment plans accurately and timely in accordance with hospital protocols and regulatory guidelines.
• Participate in student mentorship programs as appropriate, providing educational support and evaluations.
• Engage in continuous professional development through education, self-study, and active participation in professional organizations.
• Adhere to State Practice Act regulations and maintain professional licensure, completing all continuing education requirements.
• Contribute to performance improvement initiatives to enhance service delivery and outcomes.

Required Experience / Certifications / Licensure

• Degree in Physical Therapy from an accredited institution.
• 6 months to 1+ year of outpatient clinical experience.
• Current CPR certification.
• State licensure in good standing as a Physical Therapist.

Schedule

This is a full-time position with a 5x8.0 schedule.
