Long Term Care LPN

Schedule

Day shift, 6am-6pm or 10am-10pm, including every other weekend. This position requires 36 hours a week commitment.

Job Description

We are seeking a dedicated Long Term Care LPN to join our team. The ideal candidate should be willing to float between units as needed, ensuring high-quality care for our patients. Prior experience in long-term care is essential, particularly in managing patients with dementia.

Responsibilities

• Provide nursing care and support to patients in a long-term care setting.
• Monitor patient vitals and administer medications as prescribed.
• Assist with daily living activities and engaging patients in care activities.
• Collaborate with healthcare teams to develop and implement care plans.

Required Experience / Certifications / Licensure

• Minimum of 1 year of experience as a Licensed Practical Nurse (LPN).
• Must hold a current New York State LPN license.
• Basic Life Support (BLS) certification required.
