LPN - Full Time Day / Evening Rotation

Job Description

The LPN is responsible for providing skilled nursing care appropriate to the age of patients served on their assigned unit. The individual must possess the ability to assess data reflective of the patient's status and interpret the information needed to identify each patient's requirements relative to their age-specific needs. This role requires demonstrating knowledge of the principles of growth and development across the life span.

Responsibilities

• Collect data relevant to resident's physical and psychosocial condition on an ongoing basis and communicate findings to the RN and other members of the Interdisciplinary Team.
• Contribute to the creation of a developmentally age-appropriate and culturally relevant plan of resident care reflecting nursing diagnoses and interventions.
• Recommend revisions to the plan of care after evaluating resident outcomes and indicate the need for referrals.
• Maintain current knowledge and competency in nursing practice, participating in the mentoring/orientation of staff.
• Provide leadership in the professional practice setting under the direction of the RN.

Required Experience / Certifications / Licensure

• Current NY LPN Licensure
• BLS certification (AHA or RQI)
• 1 year experience serving in an LPN capacity at a long-term care facility

Schedule

This is a rotating full-time position, requiring flexibility to work day and evening shifts.

Assignment Duration

This is a 13-week contract position.
