Job Description

The Licensed Practical Nurse (LPN) performs direct and indirect resident care under the direction of licensed nursing personnel. The LPN is responsible for providing care that meets the physical, emotional, and psychosocial needs of residents in accordance with established policies and procedures.

Responsibilities

• Assist with residents' activities of daily living (ADLs) as outlined in the plan of care.
• Maintain resident environment and safety, ensuring cleanliness and prompt response to call lights and alarms.
• Meet residents' nutritional needs, including meal delivery and support with feeding.
• Support residents' rehabilitative needs as outlined in the plan of care.
• Report changes in residents' physical, emotional, and psychosocial status to the Charge Nurse in a timely manner.

Required Experience / Certifications / Licensure

• NY LPN Licensure.
• BLS certification (AHA or RQI specifically).
• One year of experience as an LPN, preferably in long-term care or as a CNA.
