Schedule
Typical schedule is Monday to Friday, 8 AM to 5 PM, with on-call and weekend rotation.
Job Description
The RN Case Manager provides skilled nursing care and case management for assigned patients in their place of residence. This role involves admitting patients onto services, coordinating and integrating services to ensure continuity of care, and facilitating the achievement of quality, clinical, and cost outcomes. The RN Case Manager creates opportunities and systems to enhance patient outcomes and employs the nursing process of assessment, planning, and implementation of care.
Responsibilities
- Provide skilled nursing care and coordinate services for patients and families based on systematic evaluations.
- Observe, assess, evaluate, and plan interventions addressing patients' physical and psychosocial statuses.
- Oversee care provided by Home Health Aides and CNAs, ensuring compliance with care plans.
- Ensure continuity of care through effective communication with interdisciplinary teams and community agencies.
- Deliver quality care and symptom management, promoting optimal health through teaching and appropriate referrals.
- Maintain accurate patient records in accordance with Agency standards and OASIS requirements.
- Participate in scheduling coordination and on-call rotation as necessary to meet patient care needs.
- Engage in quality improvement activities and attend in-service training as assigned.
Required Experience / Certifications / Licensure
Qualifications include a current RN license in the State of Montana (BSN preferred), a valid driver’s license, and reliable transportation. Preferred candidates will have 2 years of recent acute care, home care, or hospice experience, including clinical skills such as venipuncture, catheterization, wound care, and or parenteral therapy. HCHB experience is strongly preferred.
