Licensed Practical Nurse (LPN) - Evening Shift

Job Description

The primary functions of a Licensed Practical Nurse (LPN) include assisting with assessment, planning, implementing, and evaluating the care of all patients, regardless of age, during their shift. LPNs are responsible for adhering to established standards of skilled nursing practice and collaborate with the care team to provide person-centered care and services.

Responsibilities

• Administer medications and treatments under the supervision of a Registered Nurse in accordance with facility policy.
• Monitor patient vital signs, including respiration, temperature, blood pressure, and heart rate; report changes in patient/resident conditions.
• Practice ethically and professionally within scope; advocate for patients.
• Maintain clinical competence specific to care area, including required education within the stated timeframe.
• Coordinate care with the interdisciplinary team and document appropriately in EMR/Point Click Care.
• Ensure effective communication with other nursing personnel and the responsible RN regarding changes in patient conditions or observations.

Schedule

This position is a part-time evening shift, working from 3:00 PM to 11:30 PM. Travelers are expected to work every other weekend and holiday, and may be required to assist as a CNA based on staffing needs.

Required Experience / Certifications / Licensure

• Valid LPN license.
• Preferred experience in Point Click Care.
• Completion of required health assessments and immunizations including TB Test, Hepatitis B, MMR, Varicella, Tdap, and pneumococcal vaccine.
• Must pass agency background check and drug screen.
